What matters most? Supporting conversations on what matters most to older people in the last phase of life: A co-production approach

This study aims to improve support for ACP, or conversations about what matters most, by refining and adapting the existing Conversations on Living and Dying (CLaD) intervention—originally developed with OPLWAF, unpaid carers, and specialist palliative care professionals—so that it can be effectively used in community settings. 

The Care-Full Study: A systems approach to older adults with multiple long term conditions’ home-based care

This project brings health science researchers and system engineers together with unpaid carers, older people living with frailty and multi-morbidities, care advocates and care providers, to translate real-world information into a mathematical model of the complex systems and networks of factors around unpaid care.  

ALLIANCE: Enhancing the quality of living and dying with advancing frailty through integrated care partnerships: Building research capacity and capability

Our aim is to grow a research partnership of care providers that will improve the coordination of end-of-life care for older people living with advancing frailty in the community.

The PALLUP Study: Improving home-based palliative care for older people

This study will help home care services to meet the needs of older people living with severe frailty who are in their final years of life, and their families.

CLaD 2.0 - Conversations on living and dying: Facilitating advance care planning with older people living with advancing frailty 

Our aim is to co-produce resources with older people living with advancing frailty to increase ACP engagement.

Dual expert carers

This study has two aims. The first is to understand dual expert family carers’ experiences and consequent decision-making processes at a point of change in the condition of an older person living with frailty. The second aim is to facilitate the co-design of a set of guiding principles by participants, to inform a prototype tool for family carers to assist decision-making.
